Job summary

UnitedHealth Group is seeking a Home and Community Based Services Licensed Practical Nurse in Alexandria, Louisiana. In this role, you will provide direct nursing care to patients in their homes, coordinating with healthcare teams to ensure high-quality patient care.

The ideal candidate will have a current LPN licensure in Louisiana and at least 1 year of nursing experience. We offer flexible hours, competitive pay ranging from $21.70 to $32.55 per hour, and a comprehensive benefits package.

Qualifications

• Must have current and unrestricted Licensed Practical Nurse licensure in the state of practice.
• Must have current CPR certification or ability to complete within 90 days of hire.
• Must have access to a dependable vehicle or public transportation.

Responsibilities

• Provide direct nursing care as prescribed by the physician to acutely and chronically ill patients in their homes.
• Document patient visits thoroughly and completely per regulatory and payer requirements.
• Participate in the agency's care coordination process.
