• Assist community members in accessing and navigating the healthcare system with referrals.
• Provide basic health screenings and assess health risks to guide interventions.
• Support the development of health promotion programs addressing community needs.
• Collect data and maintain records of outreach activities and interventions.
• Attend meetings and trainings related to community health promotion and education.
