Job summary

Mass General Brigham Community Physicians, Inc. is seeking a Community Health Outreach role to promote and maintain health within the Brockton, Quincy and Bridgewater areas.

The position involves educating individuals, coaching groups, and helping families access essential health services through community-based activities. The role requires strong communication, teamwork, and cultural sensitivity, with a preference for bilingual English/Spanish speakers and pediatrics exposure.

Qualifications

• High School Diploma required; Associate’s/Bachelor’s degree preferred.
• Experience in community outreach, health education, or related field strongly preferred.
• Bilingual English/Spanish strongly preferred.
• Pediatrics experience strongly preferred.

Responsibilities

• Conduct community outreach and engage community members to understand health needs.
• Provide individual and group health education and coaching to community members.
• Assist community members in accessing and navigating the healthcare system, including referrals to health services.
• Provide basic health screenings and assessments to identify health risks and interventions.
• Assist in developing health promotion programs and activities to address community health needs.
• Collect data and maintain accurate records of outreach activities.
