Job summary

Home of the Innocents in Louisville, KY is seeking a Wound Care Nurse to lead the Skin and Wound Management Program at our Kosair for Kids Complex Care Center, a pediatric skilled nursing facility. You will drive evidence-based wound prevention and treatment and serve as the clinical SME for wound care across disciplines.

The role reports to the Director of Clinical Compliance, collaborates with the QAPI team and nursing leadership, and involves full-time hours with occasional evenings or

Qualifications

• Associate’s Degree in Nursing; KY RN license required.
• CWCN/CWON/CWOCN credential or willingness to obtain upon hire.
• Minimum 2 years RN experience in acute or long-term care; IDD experience preferred.
• CPR/BLS certification.
• Strong knowledge of long-term care standards and wound management regulatory requirements.

Responsibilities

• Lead all aspects of skin and wound management for residents.
• Serve as the SME for wound care; identify high-risk residents and lead surveillance.
• Collect, track, and analyze wound data; present findings to QAPI and stakeholders.
• Ensure wound protocols meet regulatory standards; respond to deficiencies and assist with corrections.
• Provide education and retraining to staff on wound prevention and best practices.
• Collaborate with Quality Improvement and Compliance to align wound management with standards.
