Job summary

Angels of Care Pediatric Home Health in Charlotte, NC is hiring Registered Nurses (RN) or Licensed Practical Nurses (LPN) to join our Pediatric Home Team, supporting medically fragile children during critical transitions from hospital to home. This Transition Nurse role focuses on safe discharges, preventing readmissions, and continuity of care.

You will provide 1:1 pediatric home care, administer medications, educate families, coordinate with care managers, and respond to emergencies.

Qualifications

• RN or LPN license (NC) required.
• Current CPR certification required.
• Ability to lift and assist pediatric patients as needed.
• Emergency response management experience preferred.

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients’ homes.
• Administer medications, treatments, and therapies.
• Support hospital-to-home transitions for high-acuity patients.
• Educate and support parents, families, and caregivers.
• Assist with discharge support and care coordination.
• Train and support permanent field nurses during transitions.
• Provide first-shift and interim coverage as needed.
• Collaborate with Clinical Care Managers and interdisciplinary teams.
• Respond appropriately in emergency situations and changes in patient condition.

Angels of Care Pediatric Home Health is hiring Registered Nurses (RN) or Licensed Practical Nurses (LPN) to join our Pediatric Home Team (Transitional Care Team), supporting medically fragile children and their families during critical transitions from hospital to home.

This role is ideal for nurses who are confident working with high-acuity patients and skilled in trach, vent, G-tube, and emergency management.

As a Transition Nurse, you'll play a critical role ensuring safe, successful discharges, preventing readmissions, providing interim patient coverage, and supporting continuity of care until permanent staffing is established.

What You’ll Do

Qualifications

Required:

• Active, unrestricted Registered Nurse (RN) or Licensed Practical Nurses (LPN) license (NC)
• Current CPR certification
• Ability to lift and assist pediatric patients as needed
• Emergency response management

Preferred

• Pediatric home health experience
• Experience with Trach care, Ventilator management, and TPN
• High-acuity pediatric patients
• Care coordination, discharge planning, or transitional care experience

Schedule & Travel

• Full-time position, with guaranteed hours
• Day and Night shifts available
• On call and Staffing coverage responsibilities as needed
• Field-based role with local travel
