Responsibilities

• Perform evaluations and develop treatment plans to meet patient goals.
• Assess treatment response and modify plans as needed.
• Maintain patient records per policies and standards.
• Attend to access to care, patient experience and quality metrics.
• Contribute to program development to meet patient/community needs.
