Job summary

Therapy 2000 in Grand Prairie, TX seeks a Pediatric Speech-Language Pathologist for PRN home-based services.

You will evaluate and treat children in their homes, develop treatment plans, collaborate with families and care teams, and support feeding therapy initiatives.

Qualifications

• Must have a current Texas Speech-Language Pathology license.
• 3+ years of pediatric SLP experience.
• Home-health experience required.
• Feeding therapy certifications (VitalStim, SOFFI, SOS, etc.) preferred.
• Innovative mindset with reliable, flexible performance.

Responsibilities

• Perform pediatric speech-language pathology evaluations using standardized tests.
• Develop and implement treatment plans aligned with physician orders.
• Create home exercise programs with families/caregivers.
• Coordinate care with other providers and disciplines in the patient’s network.
• Complete required charting, progress updates, and documentation weekly.
• Ensure compliance with agency policies and home-health regulations.

Qualifications

What you’ll need:

• A current/active license to practice Speech Language Pathology in the state of Texas.

• 3+ years as a pediatric Speech Language Pathologist
• Experience in a home-health setting.
• Experience or certifications in feeding therapy or other clinical specialties (VitalStim, SOFFI, SOS, etc.)

Responsibilities

What you’ll do:

• Perform comprehensive Speech Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• Use your clinical judgment to develop and implement relevant treatment plans aligned with physicians’ orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• Complete a minimum number of patient visits on a weekly basis as required by the agency.
• Meet deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• Meet with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• Perform all duties in compliance with agency policies and procedures.
• Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric speech language pathology.
