Job summary

Homeside Rehab in Hazleton, PA is seeking a compassionate Speech-Language Pathologist to join our pediatric home health team. You will deliver individualized, evidence-based therapy to children in their homes and enjoy schedule flexibility.

Active PA SLP license is required. Experience in home health, outpatient, or SNF is preferred. Strong communication, reliability, and the ability to work independently with families are essential. Per diem hours offered with weekday and some weekend shifts.

Qualifications

• Active Pennsylvania State Speech-Language Pathologist License is required.
• Minimum 1 year of experience in Home Health, Outpatient, or SNF settings preferred.
• Strong clinical and interpersonal communication skills.
• Ability to work independently in a home health environment.
• Reliable transportation for home visits preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on patients’ needs and goals.
• Provide evidence-based treatment for speech, language, fluency, articulation, and voice disorders.
• Maintain accurate and timely documentation of patient care.
• Collaborate with the clinical team to ensure coordinated and effective treatment.
• Communicate effectively with office staff, patients, and caregivers.
• Collaborate with healthcare professionals to ensure comprehensive care.
