Job summary

Therapy 2000 is seeking a Pediatric Speech Language Pathologist (CCC/CFY) to provide home-based therapy across Texas, including Georgetown and Round Rock. The role includes mentorship in feeding and close collaboration with families and medical teams.

You will conduct evaluations, develop evidence-based treatment plans, and coordinate care to help children reach their communication and feeding goals in the home setting.

Qualifications

• Current/active license to practice Speech Language Pathology in Texas.
• 3+ years as a pediatric Speech Language Pathologist.
• Experience in home-health setting.
• Experience or certifications in feeding therapy or other clinical specialties (VitalStim, SOFFI, SOS, etc.)

Responsibilities

• Perform comprehensive Speech Therapy evaluations using standardized tests to determine patients’ needs for therapy.
• Develop and implement relevant treatment plans aligned with physicians’ orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Collaborate and coordinate care between various providers and disciplines.
• Complete a minimum number of patient visits weekly as required by the agency.
• Meet deadlines for chart updates, progress summaries, call logs, and other paperwork.
• Meet with agency personnel to maintain understanding of expectations and standards.
• Adhere to agency policies, procedures, rules, codes of ethics, and applicable laws.

Responsibilities

What you’ll do:

• Perform comprehensive Speech Therapy evaluations, using standardized tests to determine patients’ needs for therapy.
• Use your clinical judgment to develop and implement relevant treatment plans aligned with physicians’ orders.
• Develop home exercise programs in partnership with patients’ caregivers.
• Act as a vital member of your patients’ medical neighborhoods, collaborating and coordinating care between various providers and disciplines.
• Complete a minimum number of patient visits on a weekly basis as required by the agency.
• Meet deadlines for completion of patient chart updates, progress summaries, call logs, and all other paperwork as needed.
• Meet with agency personnel as necessary to maintain understanding of expectations and standards related to delivering high-quality patient care.
• Perform all duties in compliance with agency policies and procedures.
• Adhere to all rules, regulations, codes of ethics, guidelines and federal, state and local laws applicable to home-health pediatric speech language pathology.
