Job Description:

The Emergency Dept. Pediatric Social Work Care Manager collaborates with physicians and the healthcare team to improve outcomes for patients with chronic or complex needs. The role involves educating patients and caregivers, facilitating communication, coordinating services, addressing barriers, and promoting resource allocation while balancing quality and cost.

Job Specifics

  • Pay Range Clinical: $34.06-$52.55 Non Exempt
  • Pay Range Certified: $30.94-$47.77 Non Exempt
  • Benefits Eligible: No
  • FTE: PRN
  • Shift: Days, Nights, and Weekends as needed

Job Essentials

  • Identify patient populations using screening criteria, review medical records, and conduct interviews to assess needs.
  • Develop and maintain care plans adhering to CMS COP or NCQA standards.
  • Coordinate communication among patients, families, healthcare providers, and external agencies.
  • Perform cost-benefit analyses, coordinate authorizations, and provide mental health interventions as needed.

Minimum Qualifications

  • Master's Degree in Social Work from an accredited institution.
  • Current State Licensure as a CSW or LCSW.
  • Basic computer skills and strong communication abilities.
  • Critical thinking skills.

Preferred Qualifications

  • Case Management Certification.
  • Experience in clinical care management or social work.
  • Knowledge of care management, health insurance, coding, and care guidelines.
  • Independent, motivated, adaptable, and positive attitude.

Physical Requirements:

  • Communication with others.
  • Operate computers and office equipment.
  • Read monitors and documents.
  • Remain seated or standing for extended periods.

Location: Intermountain Health Primary Children's Hospital, Salt Lake City, Utah
