Job summary

Pediatric Home Service (PHS) is seeking a compassionate nurse for day and overnight 12-hour shifts to support a pediatric patient at home near Princeton. Roles include RN or LPN responsibilities with 12-year-old client needing G-tube care, trach and ventilator support when required.

Youll provide skilled nursing, monitor conditions, document care, and collaborate with the care team to keep the child in a comfortable home setting with family involvement.

Responsibilities

• Provide skilled nursing care in home and school settings
• Assess and monitor your patient's condition and communicate changes with the care team
• Administer medications, treatments, and prescribed therapies
• Provide trach, ventilator, airway, and G-tube care as needed
• Maintain accurate documentation and support families through education
• Follow individualized care plans and maintain a safe care environment
