Job summary

Pediatric Home Service (PHS) is seeking a skilled RN or LPN for day and overnight pediatric home care near Princeton, MN. You will provide one-on-one nursing in homes and schools, assess patient conditions, administer meds, and follow care plans for a medically complex child with a g-tube.

Responsibilities

• Provide skilled nursing care in home and school settings
• Assess and monitor your patient's condition and communicate changes with the care team
• Administer medications, treatments, and prescribed therapies
• Provide trach, ventilator, airway, and G-tube care as needed
• Maintain accurate documentation and support families through education
• Follow individualized care plans and maintain a safe care environment
