Job summary

Affirmed Home Care, Inc. is seeking a Per Diem Pediatric Registered Nurse to support our expanding Pediatric Division across Mercer, Middlesex, Monmouth and Ocean counties in New Jersey.

This flexible, per diem role emphasizes field-based nursing, autonomy, and clinical oversight of pediatric home care cases. The position offers day, evening and weekend opportunities with competitive hourly pay and the chance to work with a collaborative clinical team.

Qualifications

• Active New Jersey RN license in good standing.
• BSN preferred.
• Current BLS certification.
• Minimum 2 years of pediatric home care experience.
• Hands-on pediatric clinical skills: ventilator management, tracheostomy care, G-tube/PEG care, IV infusions, wound care.
• Strong clinical assessment and documentation skills.
• Excellent communication and time-management skills.
• Ability to work independently in a field-based environment.
• Experience supervising and supporting clinical staff.

Responsibilities

• Ensure the highest quality of client care.
• Develop and update individualized care plans.
• Obtain necessary physician orders for home care cases as required by state regulations.
• Monitor and report changes in the client’s medical or mental condition to the attending physician and Agency care team.
• Supervise and support the nursing home care team.
• Conduct weekly nursing orientations.
• Conduct nursing competency assessments in the office or in the client’s home, as needed.
• Support the overall clinical needs of pediatric clients.
• Promote client safety, independence, and quality of life in the home environment.
