Shriners Children's in Sacramento is seeking a registered nurse to join our care management team as an RN Care Manager. You will advocate for patients and families, coordinate across disciplines, and ensure care aligns with treatment plans and UR/UM guidelines.

Responsibilities include intake assessments, discharge planning, medication management, and collaboration with the Medical Home to improve outcomes. California licensure and 5+ years pediatric experience preferred.

Qualifikationen

• BSN required and active California nursing license.
• Experience with disease management and application of motivational interview techniques.

• Advocates for patient and family while adhering to treatment plans and nursing/case management standards.
• Collaborates with interdisciplinary teams to meet facility goals and metrics.
• Performs intake assessments at initial visit or by phone prior to first appointment.
• Ensures coordinated care by communicating with Medical Home and other care teams.
• Provides discharge/transition assessments and develops plans to improve outcomes and reduce length of stay.
• Supports medication management and adherence through proactive follow-up with providers and families.
• Reviews service authorization and documentation to support medical necessity and appropriate level of care.
• Empowers patient/family to participate in the care plan.
