Family of Kidz is seeking a Pediatric Physical Therapist for a school-based role in Hawthorne, NY. A $1,000 sign-on bonus is offered after 90 days with active cases. The position may be fee-for-service or full-time, with a guaranteed rate for prep and training.

Responsibilities include conducting PT evaluations, coordinating with school teams, and tracking progress on IFSP/IEP goals. NY licensure and a degree in Physical Therapy are required.

Formación

• Must have degree in Physical Therapy from an accredited university.
• Current New York State Physical Therapy license.
• Bilingual is a plus.

Responsabilidades

• Administer and write up physical therapy evaluations.
• Ongoing communication with lead supervisors and coordinators regarding case management.
• Maintain professionalism with families, school teams, and other professionals.
• Maintain accurate attendance and billing logs.
• Create and maintain a schedule with assigned families and school personnel.
• Collect data on each IFSP/IEP goal and report progress quarterly.
• Provide input and attend/teleconference initial CPSE/CSE meetings and annual reviews.
