Job summary

GREEN APPLE LLC is seeking a Pediatric Physical Therapist to serve families in the Prosper/Celina area as part of our DFW division. This position supports pediatric PT evaluations, treatment plans, and home exercise programs in a home-health context.

The candidate must hold a Texas PT license, show reliability, and adapt to changing priorities while coordinating care with a multidisciplinary team to deliver high-quality patient care across visits.

Qualifications

• Active Texas PT license.

Responsibilities

• Perform comprehensive Physical Therapy evaluations using standardized tests.
• Develop and implement treatment plans aligned with physician orders.
• Develop home exercise programs with caregivers.
• Coordinate care with medical providers and other disciplines.
• Meet minimum patient visit requirements weekly.
• Maintain patient charts, progress notes, and calls in a timely manner.
• Engage with agency personnel to ensure understanding of expectations and standards.
• Adhere to agency policies and applicable laws.
