Job summary

Advocate Condell Medical Center seeks a Pediatric Physical Therapist for the Outpatient Rehabilitation West Tower in Wheeling Township, IL. Qualifications

• Doctorate in Physical Therapy from an accredited program.
• Licensed Physical Therapist in the state you work in.
• BLS for Healthcare Providers per American Heart Association.
• Pediatric Physical Therapist experience of 1 year or more.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements treatment based on evaluations and goals.
• Monitors outcomes and adjusts treatment plans, providing updates to physicians and team.
• Discharges patients with home programs and family education; coordinates referrals as needed.
• Completes required documentation including evaluations, progress notes, and discharge summaries.
