PACIFIC HEALTH SERVICES INC is seeking a dedicated Pediatric Home Health LVN to provide compassionate, in-home nursing to children in the Adelanto area. This role emphasizes family education, patient safety, and teamwork with a pediatric care team.

Responsibilities include medication administration, vital signs monitoring, and care plan collaboration with clinicians. California LVN license and CPR certification are required; pediatric/home health experience is preferred.

Qualifications

• Experience with tracheostomy and G-tube care.
• Strong communication and teamwork abilities.

Responsibilities

• Administer medications and treatments per physician orders.
• Monitor patient progress and vital signs; document findings.

• Educate families on patient care and safety at home.
• Assist with activities of daily living and mobility.
• Maintain accurate patient records and documentation.
• Respond to emergencies and provide first aid when required.

Join PACIFIC HEALTH SERVICES INC as a Pediatric Home Health LVN, where you will provide compassionate care to children in the comfort of their homes. This role is perfect for dedicated individuals looking to make a meaningful impact in the lives of young patients and their families.

PM Shift: Sunday-Thursday 3pm-11pm
Day Shift: Saturday 7am-7pm

Responsibilities:

• Administer medications and treatments as prescribed by physicians.


• Monitor and document patient progress and vital signs.


• Collaborate with healthcare professionals to develop care plans.


• Provide education and support to families regarding patient care.


• Assist with daily living activities and mobility support.


• Ensure a safe and hygienic environment for patients.


• Maintain accurate patient records and documentation.


• Respond to emergency situations and provide first aid when necessary.

Requirements:

• Current California LVN license and CPR certification.


• Experience in pediatric nursing or home health care preferred.


• Experience in tracheostomy and G-tube care.
