Job summary

Cook Children's is seeking a Nurse Practitioner or Physician Assistant for Fort Worth, TX, to provide autonomous patient care in a Neighborhood Health Center setting. You will assess, diagnose, and treat per approved protocols while addressing social determinants of health and coordinating with a multidisciplinary team.

You will support physician teams to meet empaneled patient demand, serve as a patient advocate, and help connect families to needed resources.

Qualifications

• Master’s degree required for APRN or PA role.
• Current Texas RN and APRN licenses or PA license required.
• BLS required; PALS preferred for both roles.

Responsibilities

• Autonomously perform patient assessment, diagnosis and intervention per approved protocols.
• Collaborate with multidisciplinary team to manage high-volume, complex patients.
• Incorporate social determinants of health into treatment planning and connect families to resources.
• Prescribe modalities to meet wellness and illness needs; support empaneled patients.
