The Navigator works across inpatient, post-acute, and ambulatory settings, partnering with Attendings, APPs, therapy staff, and case management to ensure safe, efficient care and optimal patient outcomes.

Qualifications

• Minimum two years of clinical nursing experience.
• One year of recent Pediatric Med/Surg experience.
• Two or more years of recent Pediatric PM&R experience.
• Bachelor's degree in Nursing.
• Flexibility to orient and work at all UCSF Medical Center locations.

Responsibilities

• Coordinate care for Pediatric PM&R and Peds Ortho patients and triage as needed.
• Perform nursing assessments and manage patients' overall care plans.
• Provide counseling and education to patients and families.
• Act as a liaison across departments to coordinate care across settings.
• Develop and implement care management plans and improve patient flow.
• Engage patients and families in shared decision-making and resource use.
