Job summary

Qualifications

• Bachelor’s, Master’s or Doctorate degree from an accredited therapy program.
• Professional experience required per state regulations.
• Current License in good standing with the State Licensing Board.
• Current CPR certification as required by state regulations.
• Successful completion of suitable screening tests with passing results.
• Adherence to accepted professional standards of practice.
• Excellent observation, oral and written communication skills.
• Reliable transportation to client sites.

Responsibilities

• Seeks and obtains information necessary to function effectively when assigned.
• Provides individual patient services and prioritizes needs for therapy.
• Assesses muscle strength, mobility, gait, ROM and rehabilitation potential.
• Participates in developing the total plan of care.
• Directs the physical therapy plan of care and documents progress.
• Re-evaluates patient progress regularly and revises the plan as needed.
• Instructs staff on therapy techniques and equipment use when applicable.
• Maintains clear clinical notes and communicates status with supervisors.
