Job summary

Angels of Care Pediatric Home Health in La Junta, CO seeks a licensed pediatric physical therapist to deliver home-based care across Colorado’s communities. You will assess, treat, and monitor progress in children, creating individualized goals and engaging families for continuity of care.

Join a supportive, clinician-led team with flexible full-time or part-time schedules, ongoing mentorship, and reimbursement for CEUs/licensing.

Qualifications

• Active, unrestricted Physical Therapist license (Colorado).
• Graduate of an accredited Physical Therapy program.
• Current CPR certification.
• Strong communication and interpersonal skills.

Responsibilities

• Provide evidence-based pediatric physical therapy in the home and community.
• Perform evaluations, assessments, and ongoing progress monitoring.
• Develop and implement individualized treatment plans and goals.
• Deliver interventions to improve mobility, strength, balance, coordination, and motor development.
• Educate and empower families and caregivers for carryover in daily routines.
• Collaborate with physicians, caregivers, and interdisciplinary teams.
• Document care in an electronic medical record system.
• Maintain compliance with all clinical and regulatory standards.

What You’ll Do

Leadership Opportunity – PT Supervisor

• We are also hiring PT Supervisors to support and oversee Physical Therapist Assistants (PTAs).
• Supervisor responsibilities include:
• Providing clinical oversight and mentorship to PTAs
• Ensuring compliance with supervision requirements and documentation standards
• Supporting quality care delivery and clinical excellence

Qualifications

Required:

• Active, unrestricted Physical Therapist license (Colorado)
• Graduate of an accredited Physical Therapy program
• Current CPR certification
• Strong communication and interpersonal skills

Preferred:

• 1+ year of pediatric experience (new grads welcome!)
• Home health experience
