Responsibilities

• Treatment planning: establish care plans matching patient and family goals.
• Intervention: administer treatments and adjust based on patient response.
• Documentation: record PT services clearly and timely per standards.
• Patient/family education: provide home programs and progress updates.
• Coordinate with health care team for discharge planning and continuity of care.
