Job summary

VNA & Hospice of the Southwest Region seeks a per diem Pediatric Physical Therapist to provide home health services in Bennington County. You will evaluate, plan, and administer medically prescribed PT in clients' homes or school settings, with supervision of students and staff as needed.

Responsibilities include scheduling, comprehensive PT assessments, developing plans of care, and coordinating with clinicians to ensure quality care.

Qualifications

• Bachelor's degree in Physical Therapy required; Master's degree preferred.
• Vermont State Physical Therapist license required.
• Valid State Driver's license required; background checks will be conducted.

Responsibilities

• Actively monitors and amends patient visit schedule to maximize productivity and patient need.
• Performs a comprehensive PT assessment with psychosocial, educational, safety and risk factor parameters.
• Develops a plan of care to promote functional independence and safety.
• Plans for and provides skilled physical therapy treatment including education for patients and caregivers.
• Coordinates patient care with the clinical team and other rehab services.
• Fills out and completes insurance paperwork for school-based services and patient care.
• Coordinates safe patient discharges and transfers; serves as Home Care PT as required.

Bachelor's degree in Physical Therapy

A DAY IN THE LIFE OF A PEDIATRIC PHYSICAL THERAPIST

Under the direction of the Kids on the Move Clinical Manager, you evaluate, plan, and administer medically prescribed or educationally relevant physical therapy treatment to clients in their homes or students in their respective educational environment. In addition, this position is responsible for clinical supervision of students, physical therapy assistants and rehabilitation aides. Responsibilities include but are not limited to:

KEY RESPONSIBILITIES

• Actively monitors and amends patient visit schedule to maximize productivity and patient need. Acts to account for new patients and changes in existing patient schedule, including but not limited to, calling patient to reschedule visits, reshuffling patient load to meet new demands, communicating recommended changes to supervisor, etc.
• Performs a comprehensive PT assessment of assigned patients including, but not limited to psychosocial, educational, safety and risk factor parameters, as evidenced by clinical documentation.
• Develops a plan of care to promote optimal functional independence and safety as documented by clinical record.
• Plans for and provides skilled physical therapy treatment including, but not limited to, therapeutic techniques, patient/care giver education, etc. assessing the patient's status and progress on each visit and ensuring quality of care as documented in the clinical record.
• As Care Manager and/or team member, coordinates patient care with other clinical team members, including but not limited to, physicians, nurses, care givers, other rehab services and students, ensuring optimal patient care and communication as noted in the clinical record.
• Accurately and completely performs functions as related to school-based services, including, but not limited to, evaluations, documentation, development of Individual Educational Plan IEP's, participation in meetings, and provision of direct and consultative services. Fills out and completes insurance paperwork in a timely manner.
• Coordinates safe patient discharges and/or transfers (or upon knowledge of) and ensures all appropriate paperwork and patient information is processed.
• Serve as a Home Care PT as required.

QUALIFICATIONS:

• Bachelor's degree in Physical Therapy from an accredited university or college required. Master's degree preferred.
• Vermont State Physical Therapist license required.
• Valid State Driver's license required. Ability to successfully pass pre-employment driver record, criminal, adult and child abuse background check/review required.
