Job summary

Children's Hospital of Philadelphia seeks a Pediatric Palliative Care RN to assess and intervene for children with chronic, complex or life-threatening conditions, supporting families across care settings.

You will contribute to interdisciplinary care, coordinate discharge planning, and help ensure service quality and patient/family satisfaction within CHOP's pediatric palliative program.

Qualifications

• Bachelor's Degree in Nursing – Required
• At least 3 years nursing experience – Required
• Pediatric care coordination/case management – Preferred
• Experience with life-threatening conditions, hospice or end-of-life care – Preferred

Responsibilities

• Participates in an Interdisciplinary approach with the Palliative Care Team in evaluating end-of-life care.
• Collaborates with primary specialty teams and families to identify discharge plans, needs and resources.
• Coordinates discharge plans for home or inpatient hospice needs (end-of-life care).
• Obtains insurance approval for end-of-life services.
• Advocates for patients and families with referral agencies.
• Maintains clinical data base for palliative care patients/families.
• Coordinates with outside agencies providing home care for outpatients on palliative care.
• Maintains documentation systems for end-of-life patients/families.
• Communicates with staff via patient care rounds.
• Establishes guidelines for patient care conferences and educational programming on end-of-life care.
• Serves as a resource to staff caring for dying patients.
