Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed.
• Designs an appropriate plan of care with goals, treatment, outcomes and discharge plan.
• Educates patients, family and caregivers using effective teaching methods.
• Evaluates clinical findings to determine diagnoses and outcomes of care.
• Performs examinations and selects reliable methods for chief complaints, screening and history.
• Screens patients to determine need for further therapy services.
• Documents evaluations and plans of care per department policy.
• Completes annual education to comply with DNV Hip and Knee Replacement Program requirements.
• Establishes a plan of care based on functional deficits and patient goals.
• Completes objective testing for ROM, strength and daily tasks.
• Communicates with referring providers on progress and recommendations.
