Responsibilities

• Collects and analyzes assessment data to determine diagnosis/issues.
• Identifies expected outcomes and develops a plan to address health promotion and restoration or health maintenance.
• Implements the identified plan and coordinates care delivery.
• Promotes health and ensures a safe environment by using standards, policies, and patient needs.
