Job summary

SpineSearch seeks a Clinical Care Coordinator - NP to join a dynamic pediatric orthopedic department in New York City. You will work with an experienced surgeon to expand ambulatory care services, develop new clinical programs, and coordinate non-operative and surgical care for children and adolescents.

Responsibilities include independent patient evaluation, follow-ups, postoperative visits, care coordination, and insurance authorizations.

Qualifications

• MSN required.
• Minimum of 3 years of nursing or related clinical experience.
• Active New York State RN and Nurse Practitioner licenses; ANCC Board Certification; BLS; DEA Certification.

Responsibilities

• Evaluates and treats pediatric and adolescent patients in outpatient and other clinical settings.
• Works independently and with attending surgeons, fellows, residents, nurses, therapists, social workers, and others.
• Provides care within the NP scope, including new patient evaluations, follow-ups, and postoperative visits.
• Coordinates care to expand ambulatory services and improve patient access.
• Obtains consultations from other specialties and follows up on recommendations.
• Prepares patient charts and orders imaging and diagnostics in EPIC.
• Assesses, diagnoses, and develops treatment plans in collaboration with physicians.
• Manages medications and non-pharmacologic treatments within scope.
• Facilitates transitions of care across settings, including admissions and discharges.
• Documents all clinical actions and communications in EPIC.
