Job summary

Qualifications

Responsibilities

• Provides in-depth clinical pharmacy services to pediatric CGT patients.
• Participates in clinic and care management activities to ensure safe medication use.
• Prescribes and adjusts medications within the collaborative practice agreement.
• Coordinates with infusion pharmacy staff and external pharmacies for seamless medication access.
• Documents care provided in patients' records and may provide discharge counseling.
