Job summary

HAHHH is seeking an Observation Assistant to provide constant observation for high-risk patients and support safe patient handling under the supervision of a Registered Nurse. You will monitor patient safety, de-escalate when needed, assist with restraints as trained, and document changes in patient behavior precisely during each shift.

Responsibilities include environmental safety checks, reporting hazards to the RN, accompanying patients to procedures, and helping with basic patient care

Qualifications

• High school graduate or equivalent.
• Must complete de-escalation training.
• Current Basic Life Support (BLS) or Heart Saver certification.

Responsibilities

• Provide constant observation of high-risk patients as assigned by the RN.
• Assist patients to undress, remove items posing safety risk, and secure belongings per unit policy.
• Monitor patient safety checks and report changes to the RN.
• De-escalate patients and assist in restraints under RN or physician direction per training and policy.
• Document patient behavior and safety checks during shifts and hand off to the next Observation Assistant.
