Job summary

Akron Children's Hospital is seeking a Nurse Program Coordinator to oversee assessment, coordination of care, discharge planning, and documentation for pediatric patients and their families. You will lead the nursing team, coordinate care for the target population, and ensure high-quality discharge planning and education.

The role requires a Bachelor's degree and RN licensure with 3-5 years of relevant experience, including pediatric care; leadership skills are preferred.

Qualifications

• Bachelor’s degree is required.
• RN Licensure is required.
• 3-5 years of relevant experience is required.
• Experience in pediatrics is required.
• Leadership experience preferred.

Responsibilities

• Assess patient/family needs and communicate changes to the care team.
• Coordinate care and discharge planning for target population.
• Document evaluation and care delivery in patient plan of care.
• Identify education needs of staff and deliver patient/family education.
• Provide nursing leadership to improve patient care as a change agent.
• Serve as a resource to nurses and healthcare providers in the practice area.
• Perform other duties as required.
