Job summary

Santa Rosa Community Health is seeking a Pediatric Nurse Practitioner to join the Patient Centered Medical Home team in Santa Rosa, CA. Under the supervision of the Medical Director, you will provide comprehensive primary care, preventive care, and health education to pediatric and adolescent patients.

Qualifications

• CA Pediatric Nurse Practitioner license required or eligible.
• Two years in a community health center setting preferred.
• Current CPR certification and ACLS if applicable (as per policy).
• Fluency in Spanish and English preferred.
• Proficiency with electronic medical records and basic MS Office.

Responsibilities

• Take complete medical histories and perform physical exams for pediatric/teen patients.
• Develop and revise treatment plans in collaboration with physicians.
• Coordinate care, including vaccines, well-child visits, and chronic disease management.
• Maintain thorough documentation in EMR and ensure continuity of care.
• Participate in quality improvement and compliance with HIPAA and clinic policies.
