Job summary

Advocate Health is seeking a Nurse Practitioner to provide comprehensive primary care within a clinic setting. You will assess, diagnose, treat, educate, refer and follow up with a panel of patients, including virtual visits, and manage chronic illnesses with a collaborative APP team.

The role requires a Master’s or Doctorate in Nursing Practice, RN license, and national certification (AGPCNP/FNP/PNP-PC/ANP).

Qualifications

• Master's or Doctorate in Nursing Practice required.
• RN license required for practice.
• Certification in area of practice from a nationally recognized certifying body required (AGPCNP, FNP, PNP-PC, or ANP).
• DEA license eligibility and prescriptive authority application required.
• BLS certification with expiry >90 days from start date.

Responsibilities

• Provide assessment, diagnosis, treatment, education, referral, and follow-up for a panel of patients.
• Manage multiple chronic illnesses and perform minimally invasive procedures.
• Recommend screenings and tests based on patient demographics.
• Work within protocols mutually agreed with the APP team.
• Identify abnormal findings early and initiate intervention.
• Participate on committees and in research as appropriate.
• Provide by-visit (including virtual) assessments, diagnosis, and treatment.
• Manage acute illnesses or chronic medication administration.
