Job summary

Qualifications

Responsibilities

• Provide nursing care using the nursing process.
• Collaborate with interdisciplinary healthcare team to coordinate patient care.
• Maintain effective communication with medical staff, nursing staff, social workers and others.
• Coordinate care with home health agencies, payors, and community resources as needed.
• Support telehealth services to assist patient needs and concerns.
• Participate in quality and safety initiatives and maintain equipment in working order.
