Job description

We are looking for a Private Duty LPN in Troy, IL

Pay Rate:

$28.00/hr-$35.00/hr

Responsibilities:

1. Assist in developing and revising the care plan.
2. Provide nursing care per the physician’s treatment plan and client’s payer authorization.
3. Monitor, record, and report treatment reactions and changes in condition to relevant personnel.
4. Provide ongoing instruction to clients, families, and healthcare team members.
5. Record patient condition and care services in electronic charts.
6. Participate in educational programs, quality assessments, and staff meetings.

Requirements:

• Active Licensed Practical Nurse or Registered Nurse License
• Problem-solving skills and ability to multi-task
• Excellent communication skills
• Positive, professional attitude
