Job summary

Gentle Care Home Services is seeking compassionate LPNs to provide one-on-one pediatric home care. You will work in patients’ homes, following individualized plans and coordinating with families and the clinical team.

Responsibilities include monitoring patient status, administering medications, and ensuring thorough documentation. Prior pediatric or home health experience is preferred, with flexible case opportunities and competitive hourly pay.

Qualifications

• Active, unrestricted Georgia LPN license.
• Current CPR certification.
• Ability to meet required background and health screening requirements.
• Pediatric or home health experience preferred.

Responsibilities

• Provide one‑on‑one nursing care according to the patient’s plan of care.
• Monitor and report changes in the patient’s condition.
• Administer medications and prescribed treatments.
• Provide G‑tube, tracheostomy, ventilator, seizure, and other skilled care when applicable.
• Assist with activities of daily living as appropriate.
• Complete accurate and timely documentation.
• Communicate with families and the Gentle Care clinical team.
• Provide compassionate support to patients and families.
