Qualifications

• Graduate of a practical nursing program or equivalent, with LPN licensure.
• Excellent communication and teamwork abilities.
• Experience with electronic medical records preferred.

Responsibilities

• Partner with providers to deliver patient-centered care.
• Administer vaccines, medications, and treatments as needed.
• Manage patient communications via phone, messages, and telehealth.
• Support preventive care and chronic condition management.
• Independently manage nurse calendars to ensure timely care.
• Contribute to quality initiatives by closing care gaps.
