Job summary

Mayo Clinic is seeking a Licensed Practical Nurse (LPN) to join our Primary Care teams in Minnesota. You will provide direct patient care across visits, phone, and telehealth, administering vaccines and treatments, and supporting preventative and chronic care management.

The role emphasizes collaboration with RNs, physicians, and allied staff, with opportunities for on-site scheduling and cross-site coverage as needed. LPN licensure in the state is required and BLS certification is expected.

Qualifications

• Graduate of a school of practical nursing with licensure.

Responsibilities

• Deliver patient-centered nursing care in primary care settings.
• Administer vaccines, medications, and treatments (injections, ear washes, wound care, etc.).
• Manage patient communications via phone, online messages, and telehealth.
• Support preventative care and chronic condition management and education.
• Independently manage nurse calendars and ensure timely patient care.
• Contribute to quality initiatives by closing care gaps and improving outcomes.
