Job summary

Community Care, Inc. is seeking a compassionate LPN to provide 1:1 private duty nursing for a 19-year-old male child in Millvale, PA. This full-time day-shift role offers one-on-one pediatric home health care, with weekends available.

You will monitor vitals, administer medications, assist with ADLs, and communicate closely with the family and care team to ensure high-quality, patient-centered care in a home setting.

Qualifications

• LPN license in PA
• Graduate of an accredited nursing program
• At least 6 months of nursing experience
• CPR certification required

Responsibilities

• Assist with activities of daily living (ADLs)
• Provide GJ-tube care/feedings
• Monitor vitals
• Administer medications safely and accurately
• Communicate clearly with care team and family
