Qualifications

• PharmD or BS Pharmacy required or equivalent from an ACPE-accredited college of pharmacy.
• Five years of professionally responsible pharmacist experience for Bachelors degree holders; inpatient hospital experience preferred for PharmD.
• BL S within 90 days; ACLS or PALS depending on site.
