Job summary

LeoForce is seeking an experienced Pediatric APRN to deliver direct skilled care to pediatric patients in palliative and hospice settings in Manhattan. The role blends in-person care four days a week with remote telehealth one day, requiring strong background in oncology, hospice or palliative care and pediatrics.

The ideal candidate has at least two years in hospice/palliative care and will lead pain management, symptom control, and family discussions about goals of care.

Qualifications

Background in oncology, hospice, or palliative care and pediatrics is required.
Nurse Practitioner with at least 2 years in hospice or palliative care.

Responsibilities

Perform patient assessments and develop care plans for palliative and hospice patients.
Lead pain and symptom management and coordinate hospice eligibility discussions.
Collaborate with treating physicians and care teams across SNF, hospital, and community settings.
Support families with goal-setting discussions and end-of-life planning.
Facilitate telehealth follow-ups from a remote work day.
