Job summary

Jobot is seeking an experienced Hospice Nurse Practitioner to provide direct skilled care to pediatric patients in palliative and hospice settings within Manhattan and via telehealth. This role requires 2+ years in Hospice/Palliative Care and a background in oncology or pediatric hospice.

The position combines four days per week in person with one remote day, includes a $15,000 sign-on bonus and a comprehensive benefits package, including a pension/retirement plan and insurance coverage.

Qualifications

• NP with at least 2 years in Hospice/Palliative Care.
• Experience with pediatrics is preferred.
• Background in Oncology or Hospice or Palliative Care required.

Responsibilities

• Perform patient assessment (cognitive and physical) and follow-up visits as needed.
• Take patient history, perform examinations, and create a care plan.
• Order diagnostic tests and formulate problem lists.
• Recommend interventions aligned with the care plan for palliation.
• Lead family meetings, goal-setting discussions, and symptom management.
• Conduct hospice eligibility reviews and educate patients/families.
• Collaborate with physicians and treatment teams; monitor responses to treatment.
• Provide patient and family education about disease progression and end-of-life care.
• Coordinate handoffs to hospice providers when hospice is elected.

We are looking to hire an experienced Nurse Practitioner to provide direct skilled care to pediatric patients in need of Palliative Care and Hospice

A background in Oncology or Hospice or Palliative Care and pediatrics is required.

This full-time role has coverage 4 days per week in Manhattan where you will see patients in the SNF, Hospital and community settings. The fifth day will be REMOTE Working from home office where there will be a Virtual Hospice Team IDT meeting, and you will do telehealth follow up visits.

NP should have at least 2 years experience in either Hospice, Palliative Care

The Nurse Practitioner will provide direct skilled nursing care to patients in the nursing home, hospital or community setting and will manage pain and symptom management, communication including goal setting discussions, and hospice eligibility review.

• NP will do patient assessment – both cognitive and physical, and may be followed by one or more visits, as appropriate to the referral questions and status of the patient and family.
• NP will be responsible for taking patient history, do appropriate examination / patient assessment, create and implement a plan of care to prevent or mitigate illness and not burden for the patient and family.
• Orders appropriate diagnostic testing
• Formulates a problem list
• Recommends interventions consistent with a care plan that advances the objectives of palliative
• care
• Provides interventions as appropriate, consistent with a care plan that advances the objectives of
• palliative care
• If appropriate, takes the lead in implementing key elements of palliative care, including family
• meetings, goal setting discussions, and symptom management.
• Performs hospice eligibility review and engages in accurate hospice-related discussions
• Collaborates with patients’ physicians and other treatment team members
• Evaluates patient's response to treatments, modifies or changes when needed.
• Provides patient and family education regarding the disease process and end of life journey
• NP offers choice when recommending hospice and assists in handoff to hospice providers if hospice election is pursued
