Qualifications

• Active Arizona Physical Therapist license.
• Current CPR certification.
• AHCCCS provider ID, if applicable.
• Article 9 certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide pediatric physical therapy services in patients’ homes.
• Evaluate patient needs and develop individualized treatment plans.
• Implement therapeutic interventions to improve mobility and independence.
• Document evaluations, sessions, progress in required timeframes.
• Educate caregivers on home exercise strategies.
