Job summary

Homeside Rehab is seeking a compassionate Pediatric Physical Therapist or PTA to join our pediatric home health team. You will deliver individualized, evidence-based therapy to pediatric patients in their homes, supporting functional goals and family engagement.

This role offers schedule flexibility, a supportive clinical team, and opportunities to expand your pediatric skills in a patient-centered home health setting.

Qualifications

• Active NY Physical Therapy License (PTA) — required
• Basic computer proficiency for electronic documentation
• Strong clinical assessment and interpersonal communication skills
• Ability to work independently in a home health environment
• Valid NY driver's license and reliable transportation for home visits
• Pediatric experience is preferred
• Minimum 1 year of experience in Home Health, Outpatient, or SNF settings

Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
• Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes
• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements
• Collaborate with the clinical team to ensure coordinated and effective treatment
• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans
