Job summary

Sentido Health in Gilmer, TX seeks a Pediatric Home Health Nurse (RN or LVN) to provide skilled nursing care in pediatric patients' homes under RN supervision and coordinate with families and the care team.

The ideal candidate will hold a Texas LVN license (or multi-state license), have pediatric/home health experience, and be able to travel locally. We offer weekly pay, generous PTO, a loyalty bonus, 401K match, and comprehensive benefits.

Qualifications

• Graduate of an accredited school of vocational nursing.
• Current Texas license as a Licensed Vocational Nurse or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC)
• One year experience as a Licensed Vocational Nurse in a clinical care pediatric setting strongly preferred.
• Home health experience preferred.

Responsibilities

• Under the direction of the RN, identifies the patient’s physical, psychosocial, and environmental needs as evidenced by documentation.
• Participates in planning and implementing care in conjunction with the RN, in accordance with the POC.
• Provides care using infection control measures that protect staff and patient.
• Ensures continuity of care through delivery of quality patient care.
• Delivers effective communication to the patient and family, team members, and other health care professionals; completes documentation in a timely manner.
• Monitors assigned cases to ensure payor requirements are met.
• Demonstrates commitment, professional growth and competency.
• Promotes the Agency philosophy and administrative policies to ensure quality of care.

Education
