Job summary

KidsCare Home Health in Portland, OR seeks a Licensed Practical Nurse (LPN) to provide and supervise in-home patient care for infants to adolescents. You will follow physician orders, perform assessments, administer medications, document care, and educate families.

The role requires an active LPN license, CPR, transport, and 1 year home health and pediatric experience are preferred. Sign-on bonuses and PTO are offered.

Qualifications

• Active LPN license in the state of Oregon.
• Current CPR certification.
• Must have reliable transportation to office and patient homes.
• One year home health experience preferred.
• One year pediatric experience preferred.

Responsibilities

• Deliver and supervise patient care in home setting according to the plan of care.
• Provide services under physician orders and relevant regulations.
• Perform comprehensive patient assessments.
• Administer prescribed medications and therapies as ordered.
• Document clear, concise clinical notes per plan of care.
• Notify RN Case Manager of changes in patient condition.
• Educate patients and families on diagnosis, treatment, and prevention.
• Collaborate with other disciplines to ensure quality care.
• Follow infection control procedures and safety guidelines.
• Comply with company policies and procedures.
