ChildServe in Johnston, IA is seeking a RN Care Coordinator to manage an assigned caseload of skilled nursing and therapy patients, conduct visits, assessments, and documentation in compliance with agency and regulatory requirements.

You will supervise clinical staff, perform in-home supervisory visits, coach, and contribute to training and performance improvement plans. This role partners with leadership and interdisciplinary teams to improve service delivery and patient outcomes.

Qualificações

• Graduate of an accredited School of Nursing; BSN preferred.
• Registered Nurse in the State of Iowa.
• Pediatric nursing certification or ability to obtain within 2 years.
• BLS/First Aid certifications within 90 days of hire.
• Two years supervisory experience or three years care coordination experience.
• Strong understanding of clinical terminology and care delivery.

Responsabilidades

• Clinical Oversight & Patient Care: manage a caseload of skilled nursing/therapy patients, perform visits, assessments and documentation.
• Staff Supervision & Development: supervise clinical staff with orientation, coaching, and evaluations; conduct in-home visits.
• Leadership & Team Collaboration: facilitate meetings and partner with leadership to improve service delivery.
• Operations & Compliance: stay current on home care regulations and ensure compliance with standards.
• Organizational Support: maintain RN Care Coordinator competency and participate in committees and initiatives.
