Job summary

Reserve Care, formerly Hospice of the Western Reserve, is seeking a pediatric-focused home health nurse. You will assess needs, deliver intermittent skilled nursing under physician direction, and support families with pain and symptom management in the patient’s home.

The role requires Ohio RN/LPN licensure and a BS or equivalent, with 3–5 years in pediatrics or hospice. You will work 40 hours across four days, with occasional evenings and weekends as needed, serving multiple counties and

  • BS or equivalent in nursing or related field.
  • 3–5 years of pediatric or hospice experience preferred.

Responsibilities

  • Assess physical, psychosocial and spiritual needs of pediatric patients and families.
  • Provide intermittent skilled nursing services under physicians' directions.
  • Monitor pain and symptoms and advise families on management.
  • Liaise with physicians and provide patient-specific information.
  • Provide emotional support to patient and family.
  • Refer and follow up with other services as appropriate.
  • Provide home care health teaching to patients and families.
  • Complete plans and reports related to home care in a timely manner.
  • Participate in care conferences, bereavement meetings and audits.
  • Plan and supervise activities of home health aides as related to patient care.

Provide palliative nursing care and support to pediatric patients and families

What You Will Do
  • Assess physical, psychosocial and spiritual needs of the patient and family.
  • Provide intermittent skilled nursing services to the patient under the patient’s physicians’ directions.
  • Monitor and evaluate pain and symptoms and provide information and assistance to the patient and family to manage those problems.
  • Serves as liaison to the patient’s physician(s) when needed; provide information to the physician based on knowledge relating to the patient’s needs.
  • Provide emotional support to the patient and family.
  • Refer and follow up patients and/or family members to other services when appropriate.
  • Provide instruction (health teaching) to the patient and family as related to home care.
  • Maintain and promptly complete all necessary plan and reports related to home care.
  • Participate in care conferences and bereavement meetings.
  • Participate in public relations activities when necessary.
  • Participate in audit review, quality assurance activities and other meetings as assigned.
  • Plan and supervise activities of HHA as related to patient care.
Success Capabilities
  • Licensed RN or LPN in the State of Ohio
  • BS or equivalent experience
  • 3-5 Years of experience in Pediatrics, or Hospice with the desire to transfer into Pediatrics
  • Experience preferred in pain and symptom management, oncology, public health, mental health
  • Experience in pediatrics and knowledge regarding normal growth and development, pediatric assessment skills, and pharmacology
  • Must work a minimum of 40 hours, four days/week
  • May at times work 4 days, 10-hour shifts based on department needs and supervisor approval
  • 1 evening per week for call coverage and 1 weekend worked per month.
  • Must have valid driver’s license, car available, and insurance coverage as required in Ohio
  • Working coverage area may include: Lorain, Cuyahoga, Lake, Ashtabula, Medina, Summit, Geauga, and Portage counties
  • All attempts will be made to place PED Nurse in the service areas closer to living residence

Conditions Of Employment
  • Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
  • Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
  • Provide an active driver’s license
  • Ability to provide proof of eligibility to work in the United States.
