Job summary

Care Angels Home Care in Brooklyn seeks a Director of Pediatric Program to lead clinical operations of our home care agency. You will oversee quality initiatives, staffing, education, onboarding, and service delivery in compliance with NYS DOH guidelines.

Responsibilities include assessing patient care needs, guiding staff, coordinating care plans, obtaining authorizations, and ensuring timely documentation and case conferences to optimize outcomes for pediatric patients.

Qualifications

• License to practice as RN or PT or OT in NY state.
• BSN required; Masters/Doctorate preferred.

Responsibilities

• Coordinate, screen, and assess home health care needs of pediatric patients.
• Ensure intake is completed within set timeframes.
• Collaborate with physicians, nurses, and discharge planners to determine level of care.
• Develop comprehensive, individualized plans of treatment per MD orders and insurance guidelines.
• Ensure DME and supplies are ordered and documented.
• Schedule and assign visits with scheduling team.
• Obtain necessary authorizations from managed care personnel.
• Conduct case conferences to ensure timely discharge planning and care coordination.

Education

RN/PT/OT license in NY

BSN required
