Job summary

HomePointe HealthCare, a Benchmark Human Services partner, is seeking full-time or part-time Registered Nurses (RNs) or Licensed Practical Nurses (LPNs) to care for a child in Angola, Indiana.

You’ll work 7 nights a week, 11 pm–7:30 am, with 1:1 patient care, flexible scheduling, and the freedom to apply your nursing skills in the home setting.

Qualifications

• Active Indiana Nursing License (RN or LPN) required.
• Valid driver's license, auto insurance, and reliable transportation.
• Willing to Travel to Nearby Clients in Need of Assistance.

Responsibilities

• Perform comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
• Develop and/or follow an individualized Plan of Care and Nursing Care Plan(s) that incorporates the individual client's specific needs.
• Provide skilled interventions with a focus for achieving realistic client outcomes within a specified time period.
• Report changes and information necessary to modify and update the Care Plan to reflect progress towards goals (outcomes).
• Participate in coordination of client care.
• Report significant changes in client status to physician and other members of the team in a timely manner.
• Maintain client records showing systematic assessment, planning intervention, and evaluation.
• Other duties as assigned.
